    *Once the form has been submitted, please call the salon to provide a credit card number to hold the appointment. Your card will not be charged unless the appointment is cancelled outside of our cancellation policy below*
    La Vida Salon and Spa's Policies:
    ​We understand that not everything is in our control so we allow for a 48 hour grace period to cancel or reschedule your appointment if needed.  Due to high demand we ask you to always confirm your appointment through automated text. If your appointment is not confirmed within 48 hours we will call you to confirm. If there is no confirmation your appointment will be canceled and considered a no show. A no show fee of 50% of the service will be charged to guests that do not call or show. Arriving later than 10 minutes past your appointment time will be considered a no show and may result in a forfeiture of appointment. All services are non-refundable once completed. By clicking I agree below, you also give LaVida Salon and Spa permission to use your photos for their website and marketing purposes.       
To save time at your appointment, please fill out the spa services consent form. Otherwise, arrive 15 minutes early to fill it out at the salon before your service.
